Proxy Access for Children (less than 12 years old) is designed to ease the responsibility of overseeing health care for your children. At any time of the day, from anywhere in the world, you can use MySutterOnline to electronically access clinical guidelines, view and print vaccination records, medication instructions and other information that can help you better manage your child's medical needs.
Proxy Access for Adults (18 and over) is designed to ease the responsibility of overseeing health care for another adult, such as an elderly parent or an adult child with disabilities. At any time of the day, from anywhere in the world, these caregivers can use MySutterOnline to electronically access clinical guidelines; medication instructions and other information that can help them better manage the patient's medical needs. Proxy Access also allows spouses and partners to share health information and tasks such as making appointments or requesting prescription renewals for one another.

How do we sign up?
For security, Proxy Access is only granted in person through SEBMF Care Centers You'll be asked to submit a Proxy Request Form for each patient. Proxy Access Request Forms are available at each SEBMF Care Centers or you may fill out the appropriate form below in advance of your visit.
Once Proxy Access is established, a link to the patient's online health record will appear in your MySutterOnline account.
What about my 12-17 year old?
Discussions regarding access to the online health record of children 12-17 years old are on-going. We anticipate tackling the complex regulations regarding protected health information for children in this age group some time in the future. We hope to develop a program that will meet the needs of families and comply with all state and federal regulations. We appreciate your patience and understanding.
